6/19 Trixie AMPS 130, PMPS 108 - Vet Visit / Blood Work Results

Hope everyone and their fur babies have had a lovely Thursday!

Trixie’s AMPS was higher this morning but I’m sure it’s because of her low PM cycle last night.

She had her first set of blood work since her diagnosis and overall things look okay. The doctor was very happy with her numbers and how regulated she is.

Her kidneys, of course are showing signs of the disease and they are concerned about her USG but she had just peed about and hour before I took her in so I am going to take her back in for an early morning test when her urine is a bit more concentrated to see how that compares.

@Wendy&Neko and @Christie & Maverick - since you are both in Canada and her results are in Canadian numbers, wondering if you could peek at her Labs and let me know your thoughts when you have the time?

I had to give her meal and PM shot 30 mins late due to the Vet visit, am I okay to stick with her usual time tomorrow am?

Hi Kat, would you mind adding the lab normal ranges to the SS? You are correct, the most concentrated urine is first pee in the morning. A USG of under 1.035 like she has would be worth continuing to monitor. Did they run an SDMA also by chance? BUN might only be a wee bit high which could be from higher protein diet, or slight dehydration. Creatinine looks normal. Higher Cholesterol is typical in diabetic cats and not something to be concerned about. Do you happen to have any previous lab work of Trixie’s? It’s often good to compare trends from previous labs.

@Marje and Gracie has a fancy calculator, so she can help too after she does a conversion and she is a wiz with labs.

No problem with giving Trixie her usual shot time tomorrow, 30 minutes late tonight won’t be an issue.

Thanks Tim! They sure did! I listened politely to the recommendation to change her food and responded with a polite, I won’t be doing that. Lol

I explained how I feel it’s more important at this time to focus on her diabetes and keeping her regulated to lessen the load on her kidneys and she agreed.

Plan is to have the USG rechecked and continue to monitor with blood work every 6 months.
 
Like Christie requested already, I need the lab ranges. There is some variation, even in Canada. For example Idexx and TrueNorth have some differences. Or was it done in house, yet another variable.

Did the vet have any comments?

I'd consider changing or at least introducing an additional food that is lower phosphorus. Most of the FF are pretty high for a kidney kitty. Most of the After Dark are OK. You've seen this set of food charts? There is one for CKD diabetes: CKD food Chart

Thank you Christie, for having a look! I’ll go in and add the normal ranges. They did not run an SDMA but I can ask them to next time I bring her in to recheck the USG. She wasn’t overly concerned with the BUN as you mentioned, it was only slightly elevated.

Unfortunately, I have nothing to compare her blood work to, she’s always been healthy and hasn’t had the need for bloodwork until now.

Hi Wendy, I will go in add those ranges shortly. The tests were run in house.

The Vet didn’t have any comments other than we need another morning urine sample to recheck the USG. She had peed about an hour before the appointment so there was not much in the bladder when they did the cysto. And they want her back in 6 months so they can monitor how things are.

Appreciate the food chart, I’ll have look and see what I can find.
 
I agree with Wendy, for sure since FF is too high in phosphorus, it would help Trixie to have low phosphorus foods. Did the vet have any concerns with the current phosphorus value? When I started looking back into it myself for Maverick, based on current creatinine over 140 and phosphorus above 1.5 mmol/L, Tanya’s CRF site shows that with IRIS as stage 2, binders would be suggested, to get her phosphorus lower than 1.5 although ideally if I recall you want it under 1.3. It may be possible if you remove the FF that diet change might be enough, but I’m not sure, so might be worth asking your vet. Boreal has low phosphorus, I found it at Global Pet Foods. The location in Shawnessy also has the Weruva steak frites, which I couldn’t find anywhere else, but both Maverick and the civvie just looked at the plate then looked at me like, you’re kidding, right? :LOL:

The vet was not concerned about her Phos levels, and I did ask about Phos binders since I used them in past with 2 of my other kitties that had CKD and she didn't think they were needed at this time. They consider her at the very edge of Stage 1 and getting that USG rechecked will confirm that diagnosis. Plan to get that test done in the next week or so.

I do plan on stopping the FF and I am going to go down the food rabbit hole this weekend and was going to give Weruva Wx a try since it seems to be quite low carb and low Phos. I will look into the ones you recommended as well - thank you! Unfortunately, Trixie has proven to be quite carb sensitive so far, so I want to try to stay under 5% if I can. Anything above that, her BG doesn't move.

Makes sense, that was also why I wondered about SDMA. Phos and creatinine while normal are on the cusp between stage 1-2 I think, per IRIS. Pet Valu has the Weruva Wx phosphorus. I got the same reaction from Jester and Mavvy on those foods, but they have a shredded and a pate style so hopefully Miss Trixie approves of one or both ;). Paws crossed food change keeps things stable, as you and I both discovered from last CRF kitties, phosphorus control makes a huge difference in the progression and overall well-being. Boreal is like Tiki, very very low carb. Good quality and I thought it smelled good lol.
