5/16 Noogi - AMPS 88, +2 95 - p'titis day #2

Yesterday

Thank you all again for the advise and support - I appreciate it so much ❤️🤗❤️🤗

Poor Noogi is still feeling very nauseous and, despite my best efforts, has eaten very little.
I tried for half an hour to get him to eat something at the time of his shot, without success, so stalled. But an hour and a half later he had a small amount of HC gravy food - so went ahead with half his usual dose of insulin. Still a bit terrifying with so little food in his system.

He is at the vet's this morning so they will monitor. He is having more fluids, and they will also do a urine test to check for ketones.

I discussed adding another anti-nausea med and an appetite stimulant, as well as syringe feeding.

While I did get some sleep I am feeling very tired and emotional. Just so worried😭

It is very emotional when they don't feel good, I always feel so helpless sometimes. I hope the vet visit helps him feel better and they get you what you need to care for your boy. Sending you all the healing vines. Give Noogi some extra cuddles for me. 🌸

My only update so far (from the vet) is that he looks more alert and that he has not yet obliged with a urine sample - so he will stay their for a few more hours.

It sounds like he’s feeling a little better, hoping that continues to improve.
You’re doing a great job getting him great care to feel better.
I’m sure he will pee when he’s good and ready (I took Ivy to the ER for that concern, she hadn’t peed. When they went to examine her on the table, she peed 😳)
I think they said it wasn’t so unusual since she was dehydrated.

I’m glad his BG remains looking good, despite it all.

Looking at Leroy's long invoice from his recent hospitalization, he got methadone, injectable Cerenia, injectable Zofran, gabapentin, Trazodone, fluids, Pro-Pectalin, Alfaxalone, Acepromazine. Basically, what the p-ititis primer thread suggests (anti nausea, pain med, fluids) with extra stuff added because Leroy is Leroy 😒

A feeding tube is probably last resort. I asked about that for Leroy and the vet didn't think it was necessary.

Hope Noogi will be feeling much better later today and has a good amount of pee that needs to come out one way or another for the vet to test🤗
 
I echo trying to get Mirtaz transdermal if you’ve got anti-nausea drugs. It also has some anti-nausea effect but maybe not as much as you might need.

I also second trying to get something like Prescription Diet a/d or Recovery. I’ve used those very successfully in the past as well as subq fluids. It can take time for pancreatitis to run its course and you’ll just need to support with subq fluids, anti-nausea and vomiting, pain relief, appetite stimulant, and whatever food you can bribe him with.

I didn't catch whether the vet gave you an appetite stimulant (e.g., mirtazapine). That may be something to discuss with the vet. However, the nausea needs to be better controlled. Like Wendy noted, it may take a combination of ondansetron and Cerenia.

You might want to try baby food. Either Gerber or Beechnut Stage 2 are fine as long as there are no veggies, especially onions or garlic. Baby food isn't nutritionally complete but it may be something Noogi will eat.

Wanted to ask specifically about Mirtaz - my vet says the transdermal is not available in this country. Only tablets. Do you think that will be satisfactory? He has not been throwing up, and I do have experience giving tablets.

The vet also mentioned that a product called Nutribound is an option, similar to Hills A/D. Not sure if you have it over there.

He did have reservations if it was suitable for diabetics though. I will ask more questions.

Looking into baby foods. And also stocking up on churus and a chicken mousse topper which he loves (also not a complete foods, but better than nothing - I did have a little success encouraging him to eat a mouthful of proper food with this on top).

He is getting subq fluids, pain relief and meds for nausea again today. I asked the vet about adding ondasetron as cerenia on its own was not working.

I am too hugely relieved (and surprised) the BG has remained steady. Hope that trend continues 🤞🤞🤞🤞🤞 (and of course anti-jinx).
 
If Noogi isn't eating anything, getting a tablet in will be hard even if you use a Pill Pocket or something. Are there compounding pharmacies in South Africa? Here in the US liquid mirtazapine is an option.

There are raw/ cooked pet food pre-mixes that could be added to baby food and non-complete foods to make them into a complete diet but I'm not sure if those are available in South Africa.

I think the main goal is to get Noogi eating and feeling better. You can worry about the blood glucose levels later. Just keep an eye on them and ketones.

I have previously given Noogs tablets without food - he is surprisingly cooperative. Just wonder if that is okay on his tummy if mostly empty.

What I do have going for me is that Noogi is very food driven and not very fussy. As soon as his nausea is under control I am sure he will eat.
I think he wanted to this morning as showed a bit of interest and licked a few times but then turned away. Clearly the nausea was just too much for him.

The mirtaz I used is transdermal and I rub it on her ear pinna, do you have that formulation, Helen?

Back in the day, Mirataz wasn't available here either, so we used the tablets, but not a full tablet. Start a low dose a work your way up. There can be some side effects. We called it Meowzipine back then, kitties get very vocal. Mirataz being absorbed through the skin comes on much slower so doesn't tend to get those side effects.

What did the vet say about ondansetron? It's a human drug, so you have to get it from the pharmacy/chemist but the vet still rights a prescription for it. It comes in oral or injectible. Injectible stings some cats, and dosing is slightly different between the two.

First thing he did when I brought him home this evening was run to his food bowl! And he has eaten a proper full meal. And is now having seconds :D

He had the same treatment today as yesterday (fluids, cerenia and dolorex). Vet said as he perked up and ate a few teaspoonfuls during the course of the day he felt no need to add extra meds at this point. Altho he did give me some gabapentin to start tomorrow night. He will also have to go in for fluids tomorrow morning.

He had a urine test today too and the other goods news is every single reading came back normal/clear/not detected. Including ketones and glucose. An enormous relief!

Vet said ondanestron is an option, he does use it. But as I just mentioned above, not given today as Noogi is eating again (hooray!).
He also said he used mirataz and another appetite stimulant (I have forgotten the name already, am so tired) which we can explore if we need to.
I did bring home some feeding syringes so I have them on hand if needed.

Crossing fingers, crossing paws, that the worst is over and he's on the mend.

So so so happy to hear this, Helen! I hope he's quickly on the rise now and you'll be past this nightmare soon, especially since his urine test came back clear too.
