Acro and CKD

Hi everyone,

I posted this in the main thread but I was advised to put it here as well! Here is the link to the original post. Any advice is appreciated. Acromegaly has beyond exhausted me and feels like a never ending battle (it really is never ending). I have read all the links about proteinuria and CKD and acromegaly and honestly I don't know what to expect going forward. I hope we see the results of the SRT soon and can figure out the cause and treatment for his proteinuria. He was tested twice in the last month(ish) and his UPCR was 1.6 and 1.7. His IM at MedVet is testing him on July 8th (she didn't specify exactly what tests we are running) and then will discuss treatment. I currently have him on Weruva Play Pate food to keep his protein and phosphorous low. He seems okay otherwise at the moment. Things have been tough being in medical school while trying to manage all of his medical issues, but we will keep pushing through and hopefully have some good news soon. Also, I've attached his most recent labs in case anyone was curious.

As far as the labs go, there is a Labs tab on the spreadsheet. You could enter the data there. I'm just mentioning it because I found it a really useful tool to track trends over time, instead of just the couple you see on the Idexx results. I see his USG is nicely concentrated, that's good to see. We've had kitties here with CKD before and sometimes it progresses really slowly. The hard thing for Neko is that she was battling heart issues too, and the treatment for the heart didn't do the kidneys any favours.

Following on about the comments on dosing lower numbers. When you can monitor, try gradually lowering the number at which you are comfortable shooting the full dose. Over time you'll gather data that'll make you more comfortable. After my initial fears, I realized that I really liked shooting lower numbers, as the cycle was a lot flatter. The 80's seemed to be her sweet spot. She'd move no more than 10 points the entire cycle, unless she bounced of course. Flat blue cycles were even better, didn't require quite as much attention. High numbers, I never had a clue what the cycle would do. Just for fun, here is Crystal's spreadsheet. Note the doses. She was on Levemir and R.
